Important Information

ITPE Health & Welfare Fund Participants
2008 Benefit Improvements
Effective January 1, 2008, Benefit Improvements will go into effect.
Contribution Rate $3.16 and above:
• Medical Plan Out of Pocket Maximum Reduces to $3,500 Per Participant, $10,500 Maximum per Family per Calendar Year
All Participants:
• Dental Plan Maximum Increases to $1,500 per Employee, $750 per Dependent per Calendar Year
• Scholarship Benefits increase to $15,000 per Year for Colleges and Universities, $7,500 for Vocational and Technical Schools

*** Note: You must fill out a New Enrollment Card When You Change Your Address, Add or Drop a Dependent. You must include Birth Certificates, Adoption Papers, Marriage License or any required Court Documents. Make sure Your Union Representative has a Copy of Your Enrollment Card.
